Chronic Care Management

Chronic Care Management (CCM) provides patients with ongoing health and wellness support, increased access to appropriate medical resources, enhanced communication with members of their care team, reduction in emergency room visits and hospitalization or readmissions, and increased engagement in their own healthcare. 

Remote Patient Monitoring

Remote patient monitoring (RPM) is a device(s) that collects a wide range of patient health data, such as vital signs, weight, and blood pressure and transmits it via the internet to a health care provider. The collected data allows a health care provider to monitor changes in patient vital signs and act accordingly. RPM encourages patients to take more control of their health.

Transitional Care Management

The Transitional Care Management (TCM) program is designed for patients who have been discharged from hospitals or other facilities. TCM involves a health care provider engaging the patient in a combination of face-to-face and telehealth visits for a period of 30 days. These health care provider visits ensure there are no gaps in patient care and prevent unnecessary patient hospitalizations.

Laboratory

Mobile Medical is able to collect lab specimens, ordered by our providers, in the privacy of your home for patients in the North East and North Central market. This eliminates the need to leave the home for lab work. Results are available in a more timely fashion and accessible through the patient portal.
